Exam tip 1

When evaluating antipsychotics, consider both their effectiveness in reducing symptoms and the severity of their side effects. A strong answer will compare typical and atypical drugs on both these points, using specific examples like Chlorpromazine and Clozapine.

Are antipsychotics a 'cure' for schizophrenia?

No, antipsychotics are not a cure. They are a form of treatment that helps to manage and reduce the severity of symptoms, particularly positive symptoms like hallucinations and delusions. Many individuals need to take them long-term to prevent relapse. Schizophrenia is considered a chronic condition that requires ongoing management, often with a combination of medication and psychological therapies.

Does CBTp try to convince patients that their experiences (e.g., hearing voices) are not real?

Not exactly. The focus of CBTp is not on denying the patient's subjective experience. Instead, it helps the patient to question their interpretation of that experience. For example, rather than arguing whether a voice is 'real', the therapist helps the patient explore alternative, less threatening explanations for the voice (e.g., 'It is a symptom of my illness') and develop coping strategies to reduce the distress it causes.

Is community care just a cheaper alternative to hospitalisation?

While cost-effectiveness is a factor, the primary driver for community care was the recognition that long-term hospitalisation (institutionalisation) was often detrimental to patients' well-being, leading to loss of skills and social isolation. Well-funded community care, with services like ACT teams, aims to provide a more humane and effective model of care that promotes recovery, independence, and a better quality of life, although underfunding can compromise its effectiveness.
